Prescriptions

Prescription Charges
Some patients can get free prescriptions. These are available through the
NHS for:

• Men and women over 60
• Pregnant women and for 12 months after birth
• Children under 16 (under 19 in full-time education)
• People with low income
• People on Income Support
• People on Family Credit
• People with certain chronic conditions

Please ask at reception for leaflet HC11,
or ask your pharmacist,
or go to the website:www.dh.gov.uk

Pre-payment Certificate

Anyone who has to pay and gets
more than 5 prescription items within 4 months,
or 14 prescription items within 12 months,
could save money with a pre-payment certificate (PPC).

You can also obtain an application form (FP95) at the website:- ww.dh.gov.uk (use the Site Index to search for Prescriptions)

Order Repeat Prescriptions

If you are taking regular medication you may be given a repeat prescription allowing you to order without seeing a doctor. Repeat prescriptions must be requested at least 48 hours in advance (excluding Saturdays and Sundays).

Once you reach your medication review date shown on the repeat prescription slip your medication will need to be reviewed by a doctor. This may mean that you need to make an appointment.

You may not request medication which is not on your repeat prescription without seeing a doctor. You will be dispensed only the quantity of medication shown on your repeat prescription.

You can request a repeat prescription in one of the following ways:

1. Post the repeat prescription slip, or drop it in to Stannary Surgery.
2. Telephone us on (01822) 615855
3. Fax your requirements to (01822) 618294. You will need to give your Patient Number.
4. By email to repeatpx@stannarysurgery.org.uk. You will need to give your Patient Number.
   Please note that we cannot guarantee your privacy if you use this method.
5. By using the on-line form. You will need your Patient Number. Click here for on-line form (graphics site)

If you do not know your Patient Number please call the Surgery.

Please allow 48 hours (excluding Saturdays and Sundays) before collecting your prescription from the Surgery.
